We are applying for a new permesso di soggiorno residenza

We are applying for a new permesso di soggiorno residenza elettiva, we were told at the Questura that an American Medicare card is not sufficient, even with current paid claims, to prove Proof of coverage. What are the alternatives? Can one purchase basic insurance that will cover the requirement as a non citizen?  Thanks for any information
