909 Filling in an Italian Cheque

Please can someone give me some guidance on how to fill in an Italian cheque. It is a 'conto estero' with the MPS bank in Italy.

I know that I need to write 'NON TRASFERIBLE' on the back or front to cross cheque it but I am not sure how to complete all of the various sections.

Any help would be much appreciated.
