9093 NHS transfer

How do I transfer from the UK to the Italian state system of welfare. I have a residence certificate for Italy + codice fiscale. However I still use the UK welfare system eg doctor, dentist when I go back to the UK between May and July each year. I assume at the moment that if I have a medical problem in Italy, I could use my EHIC and claim the cost back through the UK authorities. I have still have a house in England, which has been for sale for over a year now, so I'm still a UK resident of sorts even if Im not there 9 months of the year.Can one have dual residency if not dual entitlement to UK/Italy state benefits?
Who do I notify ? Doctor, dentist, health authority, tax authority? Is there a form such as E104 to complete. I am now retired on a private pension at age 56.

There has been alot written about this subject on previous threads, I would suggest you do a search. Would very much doubt that you can have dual health care, though not sure if your house in England would still admit you to the benefits of the English system. When we moved here permenantly our English Gp was obliged to submit a form to the NHS ,so our entitlement there would only be on an emergency basis should we return for a holiday (though cannot imagine why we would want to).
I guess the reasoning is that you cannot have it both ways!, I am sure other forum members may also have advice on this.

At the moment it seems I can't have it _either_ way, although I'm a tax payer in the EU. I really need to sort my situation out as I can't afford private health insurance, but I first need a surge in energy to deal with all the ins and outs of the situation!
