Hi Bruno,
you're quite right with number one and quite wrong with number two
1 - A dichiarazione di consenso is about personal data and if you give consent (dò il consenso) it means that the bank can use your data for purposes related to its activity (get and give informations).
You must give consent for some of the boxes, otherwise the bank will not give you the loan, other boxe are not a "must", because are related to advertising and so you're not obliged to sign them
2 - I'm not really sure about the "intestatari" part, but it seems that the bank wants to know just the names of the persons that are asking the loan and so who will sign the mortgage proposal (proposta di finanziamento).
Hope it helps
excellent, thanks very much for your help!

1. one form is entitled "Dichiarazione di Consenso" and asks me to tick a series of boxes confirming either "do il consenso" or "nego il consenso" to the various questions posed. Presumably i'm being asked to give consent or not to what i believe, but am not sure, are questions about data protection. If so, presumably i would give consent on all questions to allow the bank to ask the questions they need to ask of third parties. Could anyone tell me if that's correct and if not, what's being asked?
2. There's a line on another form, "Allegato alla Circolare" that states "intestatari_/ contestatari_ della proposta di finanziamento numero". I'm not sure what i'm supposed to fill in next to that? is it the mortgage amount?
